Together with clinicians and researchers, we are developing a new way to support the treatment conversation around Venous Thromboembolism (VTE). We created a digital experience that brings medical data and personal preferences together in a single conversation, so that clinician and patient can reach a decision: to stop, continue, or adjust treatment with blood thinners.

After a first episode of unprovoked VTE, the initial treatment is followed by an important decision point: to stop, continue, or adjust treatment with blood thinners.
The choice depends on several factors: the risk of a new clot, the risk of bleeding, and the patient's personal situation.
Existing tools focus mainly on calculating medical risks, and therefore support only part of the shared decision-making process.
We mapped out the factors that come into play during a consultation and translated them into an understandable digital experience. The biggest challenge was balance: offering enough medical depth to support the clinician's clinical judgment, while leaving room for the patient's concerns, preferences, and everyday circumstances.
Risk estimates don't tell the whole story. A patient's personal situation can be decisive for whether a treatment is workable: think of occupations with a higher risk of injury, worries about bleeding, or practical limitations around hospital visits. By giving space to these factors alongside the medical data, a richer picture emerges that clinician and patient can build on together.
